When seeking coverage and payment help from XELODA Access Solutions®, there are two forms you and your patient need to complete:
- The Statement of Medical Necessity (SMN) — A form with basic patient, insurance, and prescription information used when contacting a patient's healthcare plan to determine his or her reimbursement coverage
- The Patient Authorization and Notice of Release of Information (PAN) — A form signed and dated by your patient giving written permission for XELODA Access Solutions to discuss his or her case with you and the patient's healthcare plan
